Manufacturer and Model details can be found on the system unit for desktop computers or on the bottom of a laptop computer. On all new Dell equipment the Ethernet Address (Mac Address) and Service Tag number will be displayed on the box.

Machine Name - You can name your device on the network or a generic name will be assigned, depending on your network location.

Building and Room number must be completed. The wallbox number is located on the network point in your office. It will be labelled something like 2-A-12, B1A-C-21, D-21, Drop 12 or NUR-1-18.

When you have all the details completed, your signature is required and also the signature of your Head of Department. Send the form to the
Service Desk. Once the Service Desk receives the form, a ticket is logged. The requester will receive an email confirming the ticket number. A member of Information Solutions and Services will contact you to complete the connection process. Please note for newly purchased Dell PCs
do not power on until you have been contacted by an ISS technician.
