
The aim of the module is to enable the student to interact effectively with other care professionals in the community and healthcare settings, to improve delivery of health services and patient outcomes. It will develop insight into the importance of, and opportunity for, collaboration across nursing disciplines. It is based upon the current shift in healthcare to community-based settings for a range of patient/client services. The module will provide the students with collaborative learning opportunities to develop their professional interface skills.
This module will be delivered on-line and will run over 10 weeks. Students will be provided with electronic course materials (similar to a study guide) to guide their learning and reading. Students will work through the workbook and complete units (see below) by a specified date. A facilitator will be available to students on-line, to answer queries and provide references to further reading. Students will also attend two workshop days that is focused on the application of their learning to the clinical setting.

Unit 1: Principles of interagency Collaboration
Unit 2: Teamwork and Effectiveness
Unit 3: Advanced Communication Skills
Unit 4: Community as Context of Care
Unit 5: Discharge Planning, Empowerment, & Advocacy patient/client services.
