1st Year Midwifery Students Garda Vetting & Declaration Form Guidelines

Commencement of the programme: Commencement of the programme:
A two day orientation to the university has been arranged for Saturday 8th and Sunday 9th September, 2012. Details of the content and the venues for the two days can be found here.

The Bachelor of Nursing Science programmes will commence on Monday, September 10th, 2012 in the School of Nursing & Midwifery, Aras Moyola, with an induction day. Please access the content of day here.

Please find a map of the college with the location of the venues for your orientation.

Your uniforms will be provided for your clinical practice placements but there are other essential requirements that you will need to undertake for your placements. Please read the guidelines here for additional requirements.

Below you will find important documentation that must be completed prior to commencing the Nursing/Midwifery programme. Completed documentation must be presented on the afternoon of your orientation programme on Monday 10th September. These are important legal documents and great care is needed in completing them correctly. Ensure that you follow the instruction carefully. Delay or incorrect completion of the forms will prevent you from going on clinical placement.

Forms to be completed by candidates over 18 years of age:

D. If you have resided overseas for more than 6 months you must get a Police Clearance Certificate from those countries. The document below contains contact details from which you can seek the clearance certificates.	Overseas Clearance Form

G. Original Short Form Birth Certificatesor Authorised Copies (signed and stamped by a Garda) will also be accepted Please note Birth Certificate for ABA will not be returned). H. If you are married and plan to use your married name you will need a copy of your *Marriage Certificate*
