Charity Contribution Authorisation Form

Staff may contribute to charities listed on this form by a deduction from your salary.  Further information about the charities is available here

ASC Forms

 As of 1st Jan 2019 Additional Superannuation Contribution (ASC) has replaced PRD

ASC10 Form

The ASC10 form must be completed by an employee on commencement at NUI Galway with the exception of hourly paid employees.

Unlike PRD, ASC is based on pensionable pay, therefore this deduction is not applicable for hourly paid employees. 

 ASC12 Form

In certain circumstances a mid-year balancing mechanism should be carried out to allow an individual the benefit of the annual thresholds. In applying for a refund an employee is required to complete an ASC12 form accepting the terms and conditions of such a refund.

FOIRM ÚDARAITHE ASBHAINTE PÁROLLA

Is féidir le comhaltaí foirne síntiús a thabhairt do na carthanais atá liostaithe ar an bhfoirm seo trí aisbhaint óna dtuarastal.  Tá tuilleadh faisnéise faoi na carthanais le fáil anseo

 

Foirm ASC

Foirm ASC10 

Caithfidh fostaí an fhoirm ASC10 a líonadh nuair a thosóidh sé/sí ag obair in OÉ Gaillimh seachas i gcás fostaithe a íoctar de réir na huaire.

Ní hionann agus PRD, tá ASC bunaithe ar phá inphinsin, agus dá bhrí sin ní bhaineann an asbhaint seo le fostaithe a íoctar in aghaidh na huaire. 

Foirm ASC12

I gcásanna áirithe ba cheart meicníocht chothromaithe lár-bhliana a dhéanamh chun deis a thabhairt don duine leas a bhaint as na tairseacha bliantúla. Nuair a bheidh iarratas á dhéanamh ar aisíocaíocht beidh ar fhostaí foirm ASC12 a líonadh ag glacadh le téarmaí agus coinníollacha aisíocaíochta den sórt sin.
