Competition Terms & Conditions
1.    The SIMPle study and associated prize draws are organised by researchers employed by the Discipline of General Practice, School of Medicine, National University of Ireland Galway (hereafter referred to as “the organisers”).
2.    The SIMPle practice prize draw (hereafter “Practice Prize Draw”) are open to practices participating in the SIMPle study only (hereafter “practices”) and subject to paragraph 3 below. 
3.    Persons directly involved with the SIMPle study, members of the research team or their family members, or anyone else connected with the Prize Draws may not enter the Practice Prize Draw. 
4.    By entering the Practice Prize Draw you are accepting these terms and conditions.
5.    There is one Practice Prize Draw as part of the SIMPle study. Entry to each draw is outlined in paragraphs 6 and 7. Prizes are outlined in paragraph 8 below.
6.    A Practice can participate in the Practice Prize Draw by encouraging patients to download the UTI Diary app and adding the patient mobile phone number to the laboratory request form as outlined by the SIMPle study research team. The practice with the most patient downloads will win an iPad2. To complete the UTI Diary app, patients can download the free UTI Diary app (available for both iPhone and Android) and accept the term and conditions by providing their mobile number and selecting AGREE. The winning Practice will be assessed using the last seven digits of the patient’s mobile phone number as entered in the app compared to submitted mobile phone numbers to the SIMPle study.
7.    The Prize is one Apple iPad2 16GB.
8.    Study information will not be passed onto any third party.
9.    A practice is automatically entered into the Prize Draw when a patient whose mobile phone number is provided by the practice completes the UTI diary as requested by the SIMPle study. 10.  The closing date and time of the Practice Prize Draw is Midnight on Monday 31st March 2014. Entries received after that date and time will not be considered. The Practice Prize Draw will be observed by an independent party.   
11.  The winning practice will be notified by the organisers by phone call on or before Wednesday 30th April 2014.
12.  If a Practice rejects their prize, then the prize will be forfeited and the organisers shall be entitled to select another winner.
13.  The Prize will be sent to the winning Practice by the organisers on or before 30th May 2014.
14.  The Prize cannot be exchanged or transferred and cannot be redeemed for cash or any other prize.
15.  The organisers take no responsibility for entries that are lost, delayed, misdirected or incomplete or cannot be delivered for any technical or other reason. Proof of delivery of the entry is not proof of receipt.

17.  No purchase is necessary. 
18.  The Promoter of the Prize Draw is The SIMPle Research Team, 1 Distillery Road, The Discipline of General Practice, NUI Galway, Ireland. Any complaints regarding the Prize Draw should be sent to this address. 
19.  The organisers accept no responsibility for any damage, loss, liabilities, injury or disappointment incurred or suffered by Practices as a result of entering the Practice Prize Draw or accepting any prize.The organisers further disclaim liability for any injury or damage to participants or any other person's computer relating to or resulting from participation in or downloading any materials in connection with the Practice Prize Draw. 
20.  The organisers reserve the right at any time and from time to time to modify or discontinue, temporarily or permanently, this Practice Prize Draw with or without prior notice due to reasons outside its control (including, without limitation, in the case of anticipated, suspected or actual fraud). The decision of the organisers in all matters under its control is final and binding.
21.  The organisers shall not be liable for any failure to comply with its obligations where the failure is caused by something outside its reasonable control. Such circumstances shall include, but not be limited to, weather conditions, fire, flood, hurricane, strike, industrial dispute, war, hostilities, political unrest, riots, civil commotion, inevitable accidents, supervening legislation or any other circumstances amounting to force majeure.

22.  The Prize Draw will be governed by Irish law.
