Competition Terms and Conditions

1.    The SIMPle study and associated prize draws are organised by researchers employed by the Discipline of General Practice, School of Medicine, National University of Ireland Galway (hereafter referred to as “the organisers”).
2.    All SIMPle prize draws (hereafter “Prize Draws”) are open to adults aged 18 years and over (hereafter “participants”), presenting at their GP with symptoms of a urinary tract infection (as defined by their GP) and subject to paragraph 3 below. 
3.    Persons directly involved with the SIMPle study, members of the research team or their family members, or anyone else connected with the Prize Draws may not enter any of the Prize Draws. 
4.    By entering any of the Prize Draws you are accepting these terms and conditions.
5.    There are two Prize Draws as part of the SIMPle study. Entry to each draw is outlined in paragraphs 6 and 7. Prizes are outlined in paragraph 8 below.
6.    Participants can win an iPad2 by responding to text messages. To take part by text message participants must text YES to the first text message received and answer the 3 subsequent questions by text message. All texts are free of charge and participants can opt out by replying OPTOUT to 50100. To complete the UTI Diary app, participants can download the free UTI Diary app (available for both iPhone and Android) and accept the term and conditions by providing their mobile number and selecting AGREE. Participants who reply to all 3 texts or complete diary entries for 7 days will be automatically entered into the Prize Draw and picked at random using their mobile phone number. Participants must be part of the SIMPle study, as indicated by their GP to be eligible to enter all draws.
7.    To be in with a chance to win a One4All gift voucher participants can complete an on-line survey at www.surveymonkey.com/s/uti1and submit their mobile phone number to be entered into the Prize Draw.
8.    Prizes are an Apple iPad2 16GB and one prize of €100 worth of One4All gift vouchers.
9.    All responses received by text message, app or online survey will be anonymous and used for the purpose of the SIMPle study. Information will not be passed onto any third party.
10.  To enter the Prize Draws, you must fill in your mobile phone number and answer the questions as requested. If you have any questions about how to enter the competition or in connection with the Prize Draws, please e-mail us at simple@nuigalway.ie.
11.  The closing date and time of both Prize Draws is Midnight on Monday 31st March 2014. Entries received after that date and time will not be considered. Prize Draws will be carried out by an independent observer.   
12.  The winners will be notified by the organisers by phone call on or before Wednesday 30th April 2014. If a winner does not respond to the organisers within 14 days of being notified, then the winner's prize will be forfeited and another winner will be selected (subsequent winners are subject to the same response time).
13.  If a winner rejects their prize, then the winner's prize will be forfeited and the organisers shall be entitled to select another winner.
14.  Prizes will be sent to the winners by the organisers on or before 30th May 2014.
15.  Prizes cannot be exchanged or transferred and cannot be redeemed for cash or any other prize.
16.  The organisers retain the right to substitute prizes with another prize of similar value in the event that the original prize offered is not available. 
17.  The organisers take no responsibility for entries that are lost, delayed, misdirected or incomplete or cannot be delivered or entered for any technical or other reason. Proof of delivery of the entry is not proof of receipt.

19.  No purchase is necessary. 
20.  The Promoter of the Prize Draw is The SIMPle Research Team, 1 Distillery Road, The Discipline of General Practice, NUI Galway, Ireland. Any complaints regarding the Prize Draw should be sent to this address. 
21.  The organisers accept no responsibility for any damage, loss, liabilities, injury or disappointment incurred or suffered by participants as a result of entering the Prize Draws or accepting any prize.The organisers further disclaim liability for any injury or damage to participants or any other person's computer relating to or resulting from participation in or downloading any materials in connection with the Prize Draws. 
22.  The organisers reserve the right at any time and from time to time to modify or discontinue, temporarily or permanently, this Prize Draw with or without prior notice due to reasons outside its control (including, without limitation, in the case of anticipated, suspected or actual fraud). The decision of the organisers in all matters under its control is final and binding.
23.  The organisers shall not be liable for any failure to comply with its obligations where the failure is caused by something outside its reasonable control. Such circumstances shall include, but not be limited to, weather conditions, fire, flood, hurricane, strike, industrial dispute, war, hostilities, political unrest, riots, civil commotion, inevitable accidents, supervening legislation or any other circumstances amounting to force majeure.
24.  The Prize Draw will be governed by Irish law.
