Information for Current Students

If you wish to register with the Disability Support Service, please have your consultant or specialist fill this form, and submit it at your first meeting at the DSS offices. (Please note that forms filled by GPs will not be accepted, and that the form must include the official stamp of the consultant/specialist).

LENS (Learning and Educational Needs Summary)

As part of your registration with the Disability Support Service your educational needs will be assessed and documented in your individual LENS report. This report will give a summary of your difficulties, any academic supports required and your agreed examination supports. With your consent it will be forwarded to your lecturers for information.
