Chronic illness is described as reaching crisis proportions.  In this module you will explore how health professionals can contribute to chronic illness management.  The module will be contextualised in current policy on chronic illness management.  Particular attention will be paid to the concept of self-management and how to empower clients to self-manage.  The module is delivered using blended learning supported by workshops (equivalent to two days) and will run over 10 weeks.  The module is subdivided into four units as follows:

Unit 1: Experiencing chronic illness
Unit 2: Tackling chronic illness
Unit 3: Managing chronic illness – theory 
Unit 4: Managing chronic illness – practice

Target Market 
This module is multidisciplinary and welcomes applications from healthcare professionals from the wider multidisciplinary team (including medical doctors, nurses, midwives, physiotherapists, occupational therapists and speech and language therapists).

Prerequisite

  • Applicants must be registered healthcare professionals currently working in an Irish primary care setting.
  • Have an opportunity to care for people with chronic illnesses in his/her current work allocation.
